Birth Certificates, Death Certificates, and Genealogical Services
You may attach a copy of your ID (current driver’s license is our preference) to your online application or you may fax it to our office at 574-936-9247.
Annual Food Service Permit – $120.00 Business is open more than 6 months a year.
Prepackaged Foods Only – $60.00 Business sells prepackaged foods only.
Seasonal License 6 months or less – $60.00 Business is open less than 6 months a year.

It is essential for you and your family to be prepared in the case of an emergency. Knowing what disasters could affect your area, how to receive emergency alerts, and where to go if you and your family need to evacuate.
UPDATE:
The preliminary engineering report that was commissioned in the summer of 2019 is entering it’s final phase of completion. We are expecting the full report to be filed with the county in January of 2020. Stay tuned, we will post the full preliminary report after its acceptance by the county and have public information meetings to discuss the findings of the report.
